What is the problem representation of this patient and what is the anatomical localization for her current problem based on the clinical findings? 26 year old female married 10yrs back mother of 2 children tailor & agricultural labourer by occupation presented to casualty in a state of altered sensorium & irrelevant talk History of B/L hand small joint pains 6 yrs back Multiple joint pains 3 yrs back and diagnosed as SLE  and using treatment which she stopped 10 days ago.   Cardiology postings 52/M with valvular heart disease January 09, 2021  50 year old male patient who is a farmer by occupation presented with complaints of  1- shortness of breath since 4 years  2- palpitations since 4 years . Patient says he had  chest pain 5 years back - diffuse ,squeezing and severe  for 1 day -for which he visited a doctor and was said to have MI ? Patient experienced severe shortness of breath 4 years back -at 12 am  - sudden in onset ,severe and lasted for half hr to one hour  ,he woke up and walked for few minutes and took rest .In the mrng patient went to hospital and was said to have cardiac disease ?  Later on he was put on medications and  was on regular follow up . Shortness of breath gradually progressed over 4 years -  Since 5 months patient is  experiencing shortness of breath - on exertion ( farming ,on lifting heavy weights,walking for long distance ,climbing stairs) . patient work has been gradually declined over 5 months ,as he says he becomes dyspn

  Rishikadabbara A 32 year old patient December 28, 2020 A 32 year old male patient resident of suryapet came with  C/o pedal odema from 1 month and sob at rest from 1 day. Patient was apparently asymptotic 1 month back then developed B/l pedal edema sudden in onset,yellowish discolouration of eyes and decrease appetite with out any investigations they took herbal medications for jaundice for one day and got relieved from symptoms and patient started taking alcohol then after till yesterday. Pedal edema - B/l , pitting type , sudden in onset , extended upto knee. SOB - sudden in onset , . Palpitations , sweating and giddiness + , not associated with  chest pain ,Orthopnea and PND No H/o Fever , cough , pain abdomen , nausea , vomitings, loose motions or decreased urine output. N/k/c/o - HTN , DM II , TB , epilepsy, CVA , CAD. H/o surgery 2 years back - for right inguinal hernia. Personal history: Appetite normal Diet mixed Bowel and bladder regular Sleep adequate Alcoholic from 20 year
